Access Our Patient Portal
Click on Patient Portal link below.
Once on the Patient Portal home page, “Login” or “Create an Account.” If you are using your phone or tablet, you will first need to click on “Full Site.”
Once in the Patient Portal, on the left hand side there is a tab that says “My Health”. Find the link for Medical Forms and click it. Electronically sign the Consent to Treat and HIPAA form.
